What This Role Is All About
As a Behavioral Health Care Manager, you'll be a key part of our collaborative care team, providing services as part of the Collaborative Care Model (CoCM). Youll work closely with medical providers, psychiatric consultants, and other team members to support the mental and physical health of patients. Youll help coordinate care for patients on an assigned patient caseload along with the medical provider. You will screen and assess patients for common mental health and substance use disorders and provide brief behavioral interventions using evidence-based techniques.
