Job Description:
Key Responsibilities:
Coordinate patient care activities across multiple providers and departments.
Schedule appointments, tests, and referrals for patients.
Communicate with patients to provide education and follow-up instructions.
Track patient progress and ensure care plans are completed.
Manage documentation in the Electronic Health Record (EHR) system.
Assist in resolving insurance or authorization issues.
Collaborate with clinicians, nurses, and social workers for integrated care delivery.
EXPERIENCE/EDUCATION/QUALIFICATIONS
High School diploma or equivalent required.
Two (2) years experience in a position of similar responsibility and complexity in a medical and/or care coordination setting required.
Experience and/or training with computer applications required.
